The 2010 Section 504 Conference will provide attendees with a great notebook of information (including the latest version of the CESD 504 forms) and address a number of practical concerns faced by educators serving students with disabilities:

How can I get the CESD Section 504 forms? 


The forms are available in the conference notebook to all participants at the Fall Conference.  An electronic version (in MS Word) is also available for those who attended and provided an email address at the conference.  The electronic version includes changes made to the forms after the conference to address questions from participants. The forms are not available online.


Be sure to talk with your school attorney before making changes to your Section 504 compliance process.
